Make sure you select the download for your region:

Alternatively, just give OFTEC a ring on 0845 65 85 080 and we can send you a form in the post.
The form asks for details of the technician(s) to be registered and details of the business. Information concerning yourself and the company for which you work must be completed in full.


3. Return your application to OFTEC

You will need to send supporting information to accompany your application, such as copy of your certificate of achievement or equivalent evidence of technical competence; 2 named passport-style photographs of yourself; and your cheque or payment details. The requirements are detailed fully on the application form.

  • If you are applying online, your application is submitted automatically when you complete the process. OFTEC will contact you for any additional information that is required
  • If you have completed a paper form, you can either post it to OFTEC at the address on the form, or scan your application form and email it to us

We may contact you to request any missing information. When we have received everything we require, OFTEC will process your application and will contact you within 14 days, or 5 working days if you requested the ‘fast track’ service when you applied.
